An exciting opportunity has arisen to join Thornbury Community Services as a Community Care Coordinator. The Care Coordinator is fully responsible for completing team rotas to ensure that the care delivered is of the highest standard. You will work with all internal teams to facilitate patient centred care that enriches the quality of our patients lives. 

The ideal candidate will have: 
• Good level of basic education
• Numerate & literate
• Strong organisational skills
• Strong interpersonal skills
• Good IT skills
• Ability to manage a database & keep records up to date
• Good telephone manner
• Experience of working to deadlines and targets
• Self-motivated and driven, ability to work without supervision   
• Ability to work as part of a team                 
• Professional manner   
• Enthusiastic and committed to the business objectives of the Company   
• Ability to work in a busy environment
• Possess excellent written and spoken communication skills
• Have experience in customer service and rota management 
• Be driven to exceed set goals and targets and maximise client and nurse satisfaction
