Location: Field Based - North East/North West Region

Full time, permanent

An exciting opportunity has arisen to join Pulse Community Healthcare as a Case Manager. Reporting to the Registered Manager, the Case Manager is responsible for providing the agreed service to clients through skilled support workers and to assist individuals to live within their own homes and communities whatever their abilities through a delivery of person centred care and positive behavioural support.

This is a 'hands on’ operational role which will require time spent in clients’ homes and effective workforce management to deliver the agreed care.

The ideal candidate will have:

Good level of basic education
Experience of undertaking comprehensive risk assessments
Experience of managing client relationships
Confidently liaise with social workers, families and clients
Numerate & literate
Be able to develop person centred care plans with clients
Strong interpersonal skills
Good telephone manner
Strong people management skills
Good IT skills
Experience of managing groups of support workers
Willing to travel frequently (own car is a requirement)
Person centred approach and involvement
Service User takes priority
Non-discriminatory approach
Good knowledge and insight into the Healthcare and/or Social Care Sectors
Experience in dealing with CCG’s
We are looking for someone who can confidently be responsible for the day to day service delivery for their assigned care packages. Someone with good, innovative ideas for Service User retention. Experience in undertaking comprehensive risk assessments, develop person centred care plans and promote client independence wherever possible.
